The last time I had someone do that was on a back road near my house in the middle of the day. Just pulled up along side me laying on the horn and screaming. No idea why. Was not smoking, was not driving slow.
They may have had a full bottle of gatorade bounce off their passenger window for their trouble. They decided at that point that speeding off was a better idea than continuing to be an idiot.

Years ago, before I was into pipes and cigars I was behind a guy and every mile or so I could smell this amazing smell. Noticed a puff of pipe smoke coming out his window just as the smell hit my car. He started acting like that too, might have been because I kept following him until he was home...

I had to laugh when I read your mitten comment. I worked briefly with a guy from Lansing and anytime he needed to describe where a city was, he'd use his hand and point.

OK, maybe I'm crazy but it's happened twice in the past couple months including today on the way home from work. A guy lays on the horn while passing me and glares. Normally when I get honked at, which is rare, I know why. But these times, I was just driving, nothing out of the ordinary, not texting or otherwise distracted, etc.
The only thing I can think of is, both times I was smoking a cigar. Is it possible that two different people who are not favorably disposed to tobacco smoke were dramatically expressing their disapproval? Has this ever happened to any of you? I hope I'm not experiencing the beginning of a new trend!
The other remote possibility is the external markings on my truck. It's got a Michigan upper peninsula outline in the window - nothing objectionable about that. There's a police support blue line, again, that shouldn't be too much of a trigger. I've got a marathon-style sticker that says 7.62, but only a gun aficionado would know what that means, and probably wouldn't object. & I have a Univ of Mich theme license plate which might attract negative emotions in certain parts of Michigan, but not here in the DC suburbs.

It's been 30 plus years since I've been in DC, but it seems to me that everybody honked their horns all the time. If I remember correctly, the streets were like wagon wheels that spread out from a center circle. I think it took me three days to get the hell out of there with people honking all the time. It was a culture shock to me being from Tulsa Oklahoma as everything is laid out in mile grids here like it should be. No wonder shit doesn't get done there as everybody is driving in circles.....

DC itself is horrible for driving & I'll avoid it whenever possible. If I have to go into the city, I usually try to park in the outskirts and take the metro (subway). & in the suburban areas, the driving is the worst I've seen anywhere, and that includes some 3rd world countries I've driven in, including Italy
